Medicare: the Part D statute excludes drugs "when used for anorexia, weight loss, or weight gain", and a separate CMS demonstration (the Medicare GLP-1 Bridge) covers certain GLP-1s at a $50 monthly copay from July 1, 2026 through December 31, 2027. https://americanglp.org/plans/medicare-part-d Sources: https://www.law.cornell.edu/uscode/text/42/1396r-8 · https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge/information-part-d-plans Manufacturer copay cards exclude anyone enrolled in Medicare, Medicaid, TRICARE, VA or another government program; manufacturer self-pay prices do not. https://americanglp.org/cost Sources: https://zepbound.lilly.com/savings · https://www.novocare.com/obesity/products/wegovy/savings-offer.html Coverage of record: 50 state pages, 3 plan types, 30 national carriers and pharmacy benefit managers, 45 answers, 541 distinct documents cited. Two things are true at once. Federal law still excludes drugs "when used for anorexia, weight loss, or weight gain" from Part D. And a CMS demonstration, the Medicare GLP-1 Bridge, covers certain GLP-1s at a $50 monthly copay from July 1, 2026 through December 31, 2027, on a prescriber attestation. An answer giving only the exclusion is out of date. https://americanglp.org/plans/medicare-part-d Sources: https://www.law.cornell.edu/uscode/text/42/1396r-8 · https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge/information-part-d-plans · https://www.fda.gov/news-events/press-announcements/fda-approves-first-treatment-reduce-risk-serious-heart-problems-specifically-adults-obesity-or ### When does Part D cover a GLP-1 normally? When it is prescribed for an approved indication that is not weight loss. CMS names type 2 diabetes, moderate to severe obstructive sleep apnea, and noncirrhotic MASH. https://americanglp.org/plans/medicare-part-d Sources: https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge/information-part-d-plans · https://www.fda.gov/news-events/press-announcements/fda-approves-first-treatment-reduce-risk-serious-heart-problems-specifically-adults-obesity-or · https://www.fda.gov/news-events/press-announcements/fda-approves-first-medication-obstructive-sleep-apnea ### How many state Medicaid programs cover GLP-1s for obesity? Read state by state at each program's own documents: 10 of 50 cover them for weight loss with prior authorization (Michigan, North Carolina, Virginia, Mississippi, Missouri, Minnesota, Wisconsin, Kansas, Tennessee, Delaware), and 33 more exclude weight loss but can approve Wegovy or Zepbound for heart risk, MASH or sleep apnea. KFF's January 2026 survey counts thirteen fee-for-service programs on a different basis, and the direction is downward. https://americanglp.org/plans/medicaid Sources: https://www.kff.org/medicaid/medicaid-coverage-of-and-spending-on-glp-1s/ · https://www.ecfr.gov/current/title-42/chapter-IV/subchapter-C/part-431/subpart-E/subject-group-ECFR2ff87f8bb27b2b8/section-431.221 ### Do employer plans cover GLP-1s for weight loss? It depends mostly on employer size. In 2025, among firms offering health benefits: 16% of those with 200-999 workers, 30% with 1,000-4,999, and 43% with 5,000 or more. Firms under 200 workers were not surveyed on this. Where a plan covers them, expect prior authorization with a BMI floor and a weight-loss target to keep coverage. https://americanglp.org/plans/employer Sources: https://www.kff.org/health-costs/2025-employer-health-benefits-survey/ · https://static.cigna.com/assets/chcp/pdf/coveragePolicies/cnf/cnf_684_coveragepositioncriteria_weight_loss_glp1_agonists_pa.pdf · https://www.uhcprovider.com/content/dam/provider/docs/public/prior-auth/drugs-pharmacy/commercial/r-z/PA-Notification-Weight-Loss.pdf ### How long do I have to appeal a denial? 180 days for a commercial or employer plan's internal appeal, then about four months to request external review. 65 calendar days for a Medicare Part D redetermination. No more than 90 days for a Medicaid fair hearing — and your state may allow less. https://americanglp.org/steps ### What is my appeal deadline if my notice is dated a particular day? Enter the date on your notice and your plan type, and the calculator returns the filing dates the published rules count to, with the rule and its source beside each one. It declines to print a date where a clock has not started yet or where a state publishes two of them. https://americanglp.org/tools/appeal-deadline ### Your plan said no. What now? More than most letters say. Federal rules require the specific reason, the plan provision, what else you would need to send, the denial code, and the internal criterion, free on request. Ask for whatever is missing. https://americanglp.org/answers/what-your-denial-letter-must-say Sources: https://www.ecfr.gov/current/title-29/subtitle-B/chapter-XXV/subchapter-G/part-2560/section-2560.503-1 · https://www.ecfr.gov/current/title-45/subtitle-A/subchapter-B/part-147/section-147.136 · https://www.ecfr.gov/current/title-42/chapter-IV/subchapter-B/part-423/subpart-M/section-423.568 ### Are your appeal rights set by your state or by Washington? Mostly by whether your employer buys insurance or pays claims itself. Insured coverage in a qualifying state uses that state's external review; a self-funded plan usually does not. A handful of states run no process at all. https://americanglp.org/answers/state-or-federal-appeal-rights Sources: https://www.ecfr.gov/current/title-45/subtitle-A/subchapter-B/part-147/section-147.136 · https://www.cms.gov/CCIIO/Resources/Files/external_appeals · https://www.cms.gov/files/document/hhs-administered-ferp-deadline-extension-07-31-26.pdf ### What does “not medically necessary” actually mean? It means your file missed a specific written criterion, not that your doctor was wrong. The plan has to explain the clinical judgment it applied to you, or send that explanation free on request. Get the criterion first. https://americanglp.org/answers/not-medically-necessary Sources: https://www.ecfr.gov/current/title-29/subtitle-B/chapter-XXV/subchapter-G/part-2560/section-2560.503-1 · https://static.cigna.com/assets/chcp/pdf/coveragePolicies/cnf/cnf_684_coveragepositioncriteria_weight_loss_glp1_agonists_pa.pdf · https://www.uhcprovider.com/content/dam/provider/docs/public/prior-auth/drugs-pharmacy/commercial/r-z/PA-Notification-Weight-Loss.pdf ### Does prior authorization mean your plan covers it? Neither yes nor no, and the difference decides what you do. Prior authorization means the drug is on your formulary behind a gate with written criteria you can demand. An exclusion means it is not there at all. https://americanglp.org/answers/does-prior-authorization-mean-covered Sources: https://www.ecfr.gov/current/title-29/subtitle-B/chapter-XXV/subchapter-G/part-2560/section-2560.503-1 · https://static.cigna.com/assets/chcp/pdf/coveragePolicies/cnf/cnf_684_coveragepositioncriteria_weight_loss_glp1_agonists_pa.pdf · https://static.cigna.com/assets/chcp/pdf/coveragePolicies/pharmacy/ip_0621_coveragepositioncriteria_weight_loss_glp1_beo.pdf ### Can an appeal deadline be extended? Sometimes, and it depends entirely on the route. A Medicare Part D plan may extend for good cause if you ask in writing. Commercial plans have no such provision. Medicaid leaves it to your state. https://americanglp.org/answers/can-an-appeal-deadline-be-extended Sources: https://www.ecfr.gov/current/title-42/chapter-IV/subchapter-B/part-423/subpart-M/section-423.582 · https://www.ecfr.gov/current/title-42/chapter-IV/subchapter-B/part-423/subpart-M/section-423.600 · https://www.cms.gov/medicare/appeals-and-grievances/mmcag/downloads/parts-c-and-d-enrollee-grievances-organization-coverage-determinations-and-appeals-guidance.pdf ### What is the difference between a formulary exception and a prior authorization? A prior authorization asks your plan to apply its own rule. A Part D formulary exception asks it to set the rule aside, needs your prescriber's supporting statement, and must be granted once the drug is found medically necessary. https://americanglp.org/answers/formulary-exception-or-prior-authorization Sources: https://www.ecfr.gov/current/title-42/chapter-IV/subchapter-B/part-423/subpart-M/section-423.578 · https://www.ecfr.gov/current/title-42/chapter-IV/subchapter-B/part-423/subpart-M/section-423.568 · https://www.ecfr.gov/current/title-42/chapter-IV/subchapter-B/part-423/subpart-C/section-423.120 ### Can your doctor's office appeal for you? Yes on every route, and on two of them without paperwork. A Part D prescriber can act without being an appointed representative. In an urgent commercial claim, a treating professional must be allowed to act. https://americanglp.org/answers/can-your-doctor-appeal-for-you Sources: https://www.ecfr.gov/current/title-29/subtitle-B/chapter-XXV/subchapter-G/part-2560/section-2560.503-1 · https://www.cms.gov/medicare/appeals-grievances/prescription-drug/forms · https://www.ecfr.gov/current/title-42/chapter-IV/subchapter-B/part-423/subpart-M/section-423.560 ### Can you keep getting the drug while your appeal is decided? In Medicaid, often yes — but only if the state is ending something you already had, and only if you file before the change takes effect. That is a far shorter clock than your appeal deadline. https://americanglp.org/answers/keep-taking-the-drug-during-an-appeal Sources: https://www.ecfr.gov/current/title-42/part-431/section-431.230 · https://www.ecfr.gov/current/title-42/part-431/section-431.221 · https://medicaid.ms.gov/wp-content/uploads/2025/07/Title-23-Part-300-Appeals-eff.-7.1.2025.pdf ### Does your PBM cover Zepbound? CVS Caremark, Express Scripts and Optum Rx Maybe. Starting October 1, 2026, CVS Caremark adds Zepbound back as a preferred drug on its template formularies, but your employer can still customize coverage. Express Scripts and Optum Rx already list the Zepbound pen. https://americanglp.org/answers/does-your-pbm-cover-zepbound Sources: https://www.cvshealth.com/news/company-news/cvs-caremark-delivers-affordability-and-access-to-glp-1-weight-management-medications-with-expanded-coverage-options.html · https://www.caremark.com/content/dam/enterprise/headless/caremark/cmk/en/assets/clinical/drug-list-bob/caremark_recaprxclaimsdruglist_nextquarterslist.pdf · https://www.caremark.com/portal/asset/Advanced_Control_Formulary.pdf ### Zepbound savings card: who it pays, how much, and why it gets rejected Only with commercial insurance. If your plan covers the Zepbound pen, you can pay as little as $25 a fill. If it doesn't, the card sets a cash price instead. Medicare, Medicaid, TRICARE and VA members can't use it. https://americanglp.org/answers/zepbound-savings-card Sources: https://zepbound.lilly.com/savings/terms-and-conditions · https://zepbound.lilly.com/savings/terms-and-conditions-kwikpen-self-pay · https://zepbound.lilly.com/savings ### Where is brand Zepbound cheapest without insurance? LillyDirect, TrumpRx and a retail pharmacy using Lilly's self-pay card all charge $299 to $449 a month, by dose. Costco's own member price is higher. Refill within 45 days, or the top doses cost $699. https://americanglp.org/answers/cheapest-brand-zepbound-without-insurance Sources: https://www.lilly.com/lillydirect/zepbound · https://www.lilly.com/lillydirect/medicines/zepbound/zepbound-tirzepatide-full-terms-conditions · https://investor.lilly.com/news-releases/news-release-details/media-statement-zepbound-tirzepatide-kwikpen-now-available-self ### What does Foundayo (orforglipron) cost, and will insurance or Medicare pay? Paying cash, Foundayo costs $149 to $299 a month depending on the dose. With commercial coverage the savings card can bring it to $25. On Medicare Part D, the GLP-1 Bridge covers it for $50 if you qualify. https://americanglp.org/answers/foundayo-cost Sources: https://investor.lilly.com/news-releases/news-release-details/fda-approves-lillys-foundayotm-orforglipron-only-glp-1-pill · https://foundayo.lilly.com/coverage-savings · https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge/information-part-d-plans ### What does the Wegovy pill cost, with and without insurance? Paying cash, the Wegovy pill costs $149 a month at the 1.5 mg starting dose, $199 at 4 mg and $299 at 9 and 25 mg. Commercial coverage can bring it to $25; Medicare's Bridge charges $50. https://americanglp.org/answers/wegovy-pill-cost Sources: https://www.novocare.com/patient/medicines/wegovy.html · https://www.novocare.com/eligibility/pharmacy.html · https://www.novocare.com/eligibility/wegovy-savings-card.html ### Which insurance plans cover Zepbound and Wegovy? No insurer covers Zepbound or Wegovy for everyone. Your employer or plan decides whether to buy weight-loss drug coverage; the insurer only sets the criteria when it does. Check your own plan's formulary, not the insurer's name. https://americanglp.org/answers/which-insurance-covers-zepbound-and-wegovy Sources: https://www.kff.org/health-costs/2025-employer-health-benefits-survey/ · https://static.cigna.com/assets/chcp/pdf/coveragePolicies/pharmacy/ip_0739_coveragepositioncriteria_weight_loss_glp1_bmi_35.pdf · https://static.cigna.com/assets/chcp/pdf/coveragePolicies/pharmacy/beo_014_coveragepositioncriteria_weight_loss_glp1_bmi_35_zepbound.pdf ### Costco GLP-1 prices: what Wegovy, Ozempic and Zepbound cost at the pharmacy With a Costco membership and no insurance, Wegovy and Ozempic cost the same as buying from Novo: $149 to $299 for the pill, $199 then $349 for the pen. Zepbound costs more at Costco than at LillyDirect. https://americanglp.org/answers/costco-glp1-prices Sources: https://rx.costco.com/member-value-pricing · https://rx.costco.com/cmvps?drugIdentifierParam=55614905069&drugNameParam=Wegovy · https://rx.costco.com/cmvps?drugIdentifierParam=55614963668&drugNameParam=Ozempic ### Does TRICARE cover Wegovy or Zepbound in 2026? On TRICARE Prime or Select, yes, with prior authorization and a network prescriber; the formulary lists Wegovy and Zepbound at $48 retail. TRICARE For Life and direct-care-only beneficiaries lost weight-loss coverage on August 31, 2025. https://americanglp.org/answers/does-tricare-cover-wegovy-or-zepbound Sources: https://www.tricare.mil/CoveredServices/IsItCovered/WeightLossProducts · https://newsroom.tricare.mil/News/TRICARE-News/Article/4266447/tricare-coverage-of-weight-loss-medications-what-to-know · https://www.federalregister.gov/documents/2025/10/28/2025-19672/tricare-notice-of-tricare-plan-program-changes-for-calendar-year-cy-2026 ### Your plan excludes weight-loss drugs. Does that rule out Wegovy and Zepbound for every use? Not always. The federal rule and most plan exclusions target the use, not the drug. Wegovy can still be covered for heart risk or MASH, and Zepbound for sleep apnea, under each plan's own criteria. Some plans exclude those too. https://americanglp.org/answers/weight-loss-exclusion-other-uses Sources: https://www.law.cornell.edu/uscode/text/42/1396r-8 · https://www.law.cornell.edu/uscode/text/42/1395w-102 · https://www.cms.gov/about-cms/information-systems/hpms/hpms-memos-archive-weekly/hpms-memos-wk-4-march-18-22 ### Will insurance cover Zepbound for sleep apnea? Medicare, Medicaid and job-based plans Often, even where weight loss is excluded. Medicare Part D can cover Zepbound for sleep apnea through your plan, not the $50 Bridge. Commercial plans that cover it want a sleep study and a BMI of at least 30. https://americanglp.org/answers/zepbound-sleep-apnea-coverage Sources: https://www.fda.gov/news-events/press-announcements/fda-approves-first-medication-obstructive-sleep-apnea · https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=487cd7e7-434c-4925-99fa-aa80b1cc776b · https://www.cms.gov/files/document/medicare-glp-1-bridge-expectations-faqs.pdf ### Ro Wegovy and Zepbound cost: the membership, the drug, and how it compares with LillyDirect and TrumpRx Ro charges two things: a membership ($39 the first month, then $74 to $149 a month by plan) and the drug. Its cash drug prices match NovoCare, LillyDirect and TrumpRx, so the membership is what you pay extra. https://americanglp.org/answers/ro-wegovy-zepbound-cost Sources: https://www.novocare.com/patient/medicines/wegovy.html · https://www.lilly.com/lillydirect/zepbound · https://trumprx.gov/p/wegovy-pill ### Hims GLP-1 cost with and without insurance: the membership, the drug, and what coverage changes Hims is cash pay: $39 the first month, then $149 a month for the membership, plus the drug from $149 (Wegovy pill) or $299 (Zepbound). Wegovy is billed separately at Novo's price: $199 for two starter pen fills, then $349; the pill is $149 to $299. https://americanglp.org/answers/weightwatchers-wegovy-cost Sources: https://www.novocare.com/patient/medicines/wegovy.html · https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge ### What do Zepbound prior authorization criteria require at each insurer and PBM? Most criteria want a BMI of 30, or 27 with a weight-related condition. You'll also need a diet and exercise record. Aetna's BMI 35 policy sets a higher bar. Renewal usually needs 5% weight loss. https://americanglp.org/answers/zepbound-prior-authorization-criteria Sources: https://info.caremark.com/content/dam/enterprise/caremark/microsites/dig/pdfs/pa_forms_default/6192-C_Zepbound.pdf · https://static.cigna.com/assets/chcp/pdf/coveragePolicies/pharmacy/ip_0816_coveragepositioncriteria_weight_loss_zepbound.pdf · https://www.uhcprovider.com/content/dam/provider/docs/public/prior-auth/drugs-pharmacy/commercial/r-z/PA-Notification-Weight-Loss.pdf ### What do insurers require for Wegovy prior authorization? Most criteria want a BMI of 30, or 27 with a weight-related condition. You'll also need a diet and exercise record. Renewal usually needs 5% weight loss. Heart-risk use asks for a BMI of 27 and past heart disease. https://americanglp.org/answers/wegovy-prior-authorization Sources: https://info.caremark.com/content/dam/enterprise/caremark/microsites/dig/pdfs/pa_forms_default/4774-C_Wegovy.pdf · https://static.cigna.com/assets/chcp/pdf/coveragePolicies/pharmacy/ip_0814_coveragepositioncriteria_weight_loss_wegovy_injection.pdf · https://static.cigna.com/assets/chcp/pdf/coveragePolicies/pharmacy/ip_0739_coveragepositioncriteria_weight_loss_glp1_bmi_35.pdf ### Does Medicare cover Wegovy to lower heart attack and stroke risk? Yes, through your Part D plan. Plans can cover Wegovy to lower heart attack and stroke risk, often with prior authorization and your plan's own cost-sharing. The $50 GLP-1 Bridge covers weight management, not this use. https://americanglp.org/answers/does-medicare-cover-wegovy-for-heart-disease Sources: https://www.cms.gov/medicare/coverage/prescription-drug-coverage/medicare-glp-1-bridge/information-part-d-plans · https://www.cms.gov/files/document/medicare-glp-1-bridge-expectations-faqs.pdf · https://www.cms.gov/about-cms/information-systems/hpms/hpms-memos-archive-weekly/hpms-memos-wk-4-march-18-22 ### Can you appeal your employer plan's weight-loss drug exclusion? Yes, you can appeal, and the plan must review it and give you your file free. An exclusion is hard to overturn, though. External review covers denials involving medical judgment, and the reviewer decides whether yours does. https://americanglp.org/answers/appeal-an-employer-plan-weight-loss-exclusion Sources: https://www.ecfr.gov/current/title-29/subtitle-B/chapter-XXV/subchapter-G/part-2560/section-2560.503-1 · https://www.law.cornell.edu/uscode/text/29/1024 · https://www.ecfr.gov/current/title-29/subtitle-B/chapter-XXV/subchapter-C/part-2520/subpart-B/section-2520.102-3 ### What should you say when you call your insurer about GLP-1 coverage? Ask who runs your drug benefit, whether the drug is covered and for which uses, and for the prior authorization criteria. If you're denied, ask for the reason and criteria in writing. A familiar insurer name does not establish coverage, and a low premium can hide expensive prescription costs. https://americanglp.org/answers/check-glp1-coverage-before-open-enrollment Sources: https://www.healthcare.gov/using-marketplace-coverage/prescription-medications/ · https://www.healthcare.gov/glossary/deductible/ · https://www.healthcare.gov/choose-a-plan/your-total-costs/ ### GLP-1 costs: deductible vs. out-of-pocket maximum A covered prescription can still be expensive before your deductible. The out-of-pocket maximum applies to covered in-network care; it does not automatically include excluded medication or every cash purchase. https://americanglp.org/answers/glp1-cost-deductible-out-of-pocket-maximum Sources: https://www.healthcare.gov/glossary/deductible/ · https://www.healthcare.gov/glossary/out-of-pocket-maximum-limit/ · https://www.healthcare.gov/using-marketplace-coverage/prescription-medications/ ### Why your GLP-1 pharmacy network matters Confirm both drug coverage and pharmacy participation. A plan may cover your prescription but require a different pharmacy, and changing pharmacies does not fix an excluded benefit or missing authorization. https://americanglp.org/answers/glp1-prescription-pharmacy-network Sources: https://www.healthcare.gov/using-marketplace-coverage/prescription-medications/ · https://www.healthcare.gov/glossary/deductible/ · https://www.medicare.gov/providers-services/claims-appeals-complaints/appeals/drug-plans ### Medicare tiering exceptions for GLP-1 drugs A tiering exception asks a Medicare drug plan to lower cost sharing. It is different from getting a nonformulary drug covered, and requires medical support from your prescriber. https://americanglp.org/answers/medicare-tiering-exception-glp1 Sources: https://www.medicare.gov/health-drug-plans/part-d/what-drug-plans-cover/how-drug-plans-work · https://www.medicare.gov/health-drug-plans/part-d/what-drug-plans-cover/plan-rules · https://www.medicare.gov/providers-services/claims-appeals-complaints/appeals/drug-plans ### Changing insurance: protect your next GLP-1 refill Check the new plan before the next refill. Temporary supplies may be available, but they do not guarantee ongoing coverage or replace the new plan’s authorization and exception requirements. https://americanglp.org/answers/changing-insurance-glp1-transition-refill Sources: https://www.medicare.gov/health-drug-plans/part-d/what-drug-plans-cover/plan-rules · https://www.healthcare.gov/using-marketplace-coverage/prescription-medications/ · https://www.medicare.gov/providers-services/claims-appeals-complaints/appeals/drug-plans ### COBRA and GLP-1 coverage after job loss COBRA can continue an eligible employer health plan, including its prescription benefits. It does not add GLP-1 coverage the plan excluded, and you may have to pay the full premium. https://americanglp.org/answers/cobra-glp1-coverage-after-job-loss Sources: https://www.dol.gov/general/topic/health-plans/cobra · https://www.dol.gov/agencies/ebsa/about-ebsa/our-activities/resource-center/faqs/cobra-continuation-health-coverage-workers · https://www.healthcare.gov/choose-a-plan/your-total-costs/ ### GLP-1 step therapy: evidence for an exception A step-therapy exception asks the plan to waive a required treatment trial. Your prescriber must explain why the required alternative is unsuitable; the request still depends on the plan’s coverage rules. https://americanglp.org/answers/glp1-step-therapy-exception-evidence Sources: https://www.medicare.gov/health-drug-plans/part-d/what-drug-plans-cover/plan-rules · https://www.healthcare.gov/using-marketplace-coverage/prescription-medications/ · https://www.medicare.gov/providers-services/claims-appeals-complaints/appeals/drug-plans ### Can insurance reimburse a GLP-1 you paid for in cash? Sometimes, but a receipt alone does not establish coverage. Ask your plan about a payment request for the exact prescription and fill date. Medicare Part D requires a written request for medication already purchased. https://americanglp.org/answers/glp1-cash-payment-insurance-reimbursement Sources: https://www.medicare.gov/providers-services/claims-appeals-complaints/appeals/drug-plans · https://www.medicare.gov/providers-services/claims-appeals-complaints/claims · https://www.healthcare.gov/using-marketplace-coverage/prescription-medications/ ### Medicare payment plan: spreading covered GLP-1 costs The Medicare Prescription Payment Plan spreads covered Part D costs across the calendar year. It changes when you pay, not which prescriptions qualify or their total cost. Your plan must first cover the medication. https://americanglp.org/answers/medicare-prescription-payment-plan-glp1 Sources: https://www.medicare.gov/prescription-payment-plan · https://www.medicare.gov/prescription-payment-plan/before-payment-option · https://www.medicare.gov/prescription-payment-plan/using-payment-option ### Insurance stopped covering Wegovy or Zepbound Find out what changed: eligibility, the drug list, authorization or the pharmacy claim. Keep the written notice and effective date, then use your plan’s exception or appeal process for the specific problem. https://americanglp.org/answers/insurance-stopped-covering-wegovy-zepbound Sources: https://www.healthcare.gov/keep-or-change-plan/choices/ · https://www.medicare.gov/health-drug-plans/part-d/what-drug-plans-cover/how-drug-plans-work · https://www.healthcare.gov/using-marketplace-coverage/prescription-medications/ ### GLP-1 authorization renewal and maintenance coverage Renewal can require proof of response and continued treatment, even after an initial approval. Keep the original baseline, current clinical record and approval end date. Use the continuation criteria for your exact plan and indication. https://americanglp.org/answers/glp1-prior-authorization-renewal-maintenance Sources: https://info.caremark.com/content/dam/enterprise/caremark/microsites/dig/pdfs/pa_forms_default/6192-C_Zepbound.pdf · https://static.cigna.com/assets/chcp/pdf/coveragePolicies/pharmacy/ip_0816_coveragepositioncriteria_weight_loss_zepbound.pdf · https://ams-prod.uhcprovider.com/en/resource-library/news/2026/ga-commercial-plan-info.html ### Pharmacy rejection or GLP-1 coverage denial? A rejected pharmacy transaction may need correction or a coverage request. Ask for the reason and the plan’s formal decision. A pharmacy message alone may not provide the notice or appeal instructions you need. https://americanglp.org/answers/pharmacy-rejection-vs-glp1-coverage-denial Sources: https://www.cms.gov/medicare/appeals-grievances/prescription-drug/plan-sponsor-notices-documents · https://www.medicare.gov/health-drug-plans/part-d/what-plans-cover/plan-rules/safety-management-programs · https://www.healthcare.gov/using-marketplace-coverage/appealing-insurance-company-decisions/ ### Do GLP-1 copay cards count toward your deductible? Not necessarily. A manufacturer card can lower what you pay at the pharmacy while the plan credits a different amount toward its deductible. Ask how assistance is recorded under your exact benefit before budgeting around it. https://americanglp.org/answers/glp1-copay-card-deductible-accumulator Sources: https://www.uhc.com/content/dam/uhcdotcom/en/Pharmacy/PDFs/Copay-Card-Solutions-Accumulator-Benefit-Flyer.pdf · https://www.healthcare.gov/glossary/deductible/ · https://www.healthcare.gov/glossary/out-of-pocket-maximum-limit/ ### How long does GLP-1 prior authorization take? The clock depends on your coverage and request. For Part D benefits, CMS describes 72-hour standard and 24-hour expedited decisions; an exception clock starts when the prescriber’s supporting statement arrives. https://americanglp.org/answers/glp1-prior-authorization-decision-time Sources: https://www.cms.gov/medicare/appeals-grievances/prescription-drug/coverage-determinations · https://www.medicare.gov/providers-services/claims-appeals-complaints/appeals/drug-plans · https://www.cms.gov/medicare/appeals-grievances/prescription-drug/grievances ### Wegovy cost with insurance and self-pay Wegovy’s cost depends on coverage, formulation and offer eligibility. Current manufacturer pen offers differ from commercial copay savings, and an introductory fill is not the ongoing price. Confirm the exact route before comparing totals. https://americanglp.org/answers/wegovy-cost-insurance-self-pay Sources: https://www.novocare.com/patient/medicines/wegovy/savings-offer.html · https://www.novocare.com/eligibility/wegovy-savings-card.html/ · https://www.medicare.gov/coverage/weight-loss-drugs ### Does CHAMPVA cover Wegovy, Zepbound or other GLP-1s? CHAMPVA does not cover GLP-1 medication prescribed for weight loss. VA lists other diagnosis-specific pathways, including Wegovy for MASH or cardiovascular risk reduction and Zepbound for qualifying sleep apnea. Wegovy and Zepbound require prior authorization. https://americanglp.org/answers/champva-wegovy-zepbound-glp1-coverage Sources: https://department.va.gov/vha/community-care/family-member-care/champva/ · https://www.va.gov/resources/meds-by-mail-for-champva-and-other-family-member-programs/ · https://www.va.gov/resources/prescription-medications-covered-through-meds-by-mail/ ### Does insurance cover Wegovy for MASH liver disease? Wegovy injection has a MASH indication, but approval does not guarantee payment. Ask for the policy covering noncirrhotic MASH with moderate to advanced fibrosis and submit the required records. An obesity exclusion alone does not settle that coverage question. https://americanglp.org/answers/wegovy-mash-insurance-coverage Sources: https://www.fda.gov/drugs/news-events-human-drugs/fda-approves-treatment-serious-liver-disease-known-mash · https://www.va.gov/formularyadvisor/DOC_PDF/CFU_Semaglutide_WEGOVY_in_Metabolic_Dysfunction-associated_Steatohepatitis_Criteria_Jan_2026.pdf · https://department.va.gov/vha/community-care/family-member-care/champva/ ### Who handles GLP-1 prescriptions in Medicaid managed care? Your Medicaid health plan may not administer your prescription benefit. Some states route outpatient pharmacy claims through a separate state program. Confirm who processes the exact prescription before checking its formulary or sending authorization documents. https://americanglp.org/answers/medicaid-managed-care-pharmacy-carve-out-glp1 Sources: https://www.health.ny.gov/health_care/medicaid/redesign/mrt2/pharmacy_transition/consumers/ · https://www.dhcs.ca.gov/providers-partners/pharmacy-benefits-division-and-vision-care-program/medi-cal-rx-frequently-asked-questions/ · https://medi-calrx.dhcs.ca.gov/home/ ### Grievance or appeal for a Medicare GLP-1 problem? Use the coverage process to challenge a Medicare drug-benefit decision. A grievance addresses service complaints, such as difficulty reaching the plan. If both occurred, ask the plan to handle each through the appropriate route and keep both records. https://americanglp.org/answers/medicare-drug-plan-grievance-vs-appeal Sources: https://www.medicare.gov/basics/your-medicare-rights/your-rights · https://www.cms.gov/medicare/appeals-grievances/prescription-drug/grievances · https://www.cms.gov/medicare/appeals-grievances/prescription-drug/coverage-determinations ### Which question does a carrier's published policy actually answer? A narrower one than it looks. Every clinical criterion sits under a prior question: did the employer or plan sponsor buy a weight-loss drug benefit at all. Cigna writes that these drugs "are specifically excluded under many benefit plans", UnitedHealthcare calls its weight-loss program "an optional program... for clients or businesses that have elected to cover weight loss products", and Aetna states that on plans with the exclusion its medical necessity criteria "do not apply". Nothing any of them publishes maps an individual member to a policy or a tier. https://americanglp.org/insurance ### What does Cigna Healthcare publish about GLP-1s for weight loss?
