Wisconsin rebuilt this benefit rather than trimming it. On one date it created a drug class that had not existed before, made two drugs preferred inside it, and stopped covering eight anti-obesity drugs it had been paying for. Both halves are in the same bulletin, which is unusual and makes the change easy to date precisely.

The result is a narrower door than the drug list suggests. Two products are the preferred route for adult weight loss. Several familiar names are gone entirely. And Wegovy, which most people would expect to be the obvious candidate, is not an adult weight-loss route in Wisconsin at all — it is approvable for other reasons, and for adolescents, but not for an adult who wants to lose weight.

Wisconsin is also blunt about the boundary of the class. A weight management agent that is not in the new class and has no separate criteria of its own is a noncovered service, which is a stronger statement than simply being absent from a list.

One point runs the opposite way from most states on this site: Wisconsin says these drugs stay covered for people who also have Medicare Part D coverage.

What we could verify for Wisconsin

The class itself is new, and approval is required even for the preferred drugs in it. The bulletin states: "Effective July 1, 2026, ForwardHealth will add weight management agents as a new drug class to the PDL. Foundayo and Zepbound will become preferred drugs in the weight management agents drug class. PA will be required for all weight management agents, including preferred weight management agents. Weight management agents that are not included in this PDL drug class or do not have separate established clinical criteria are noncovered services." It adds that requests "will only be approved for one weight management agent per member at a time."

The entry test is a body mass index floor with a two-risk-factor alternative, and the risk factors are enumerated rather than left open. Approval requires "The member has a body mass index (BMI) greater than or equal to 30" or "The member has a BMI greater than or equal to 27 but less than 30 and has two or more of the following risk factors: Dyslipidemia, Hypertension, Sleep apnea, Type 2 diabetes mellitus, Cardiovascular disease supported by a history of at least one of the following" — the bulletin then lists myocardial infarction, coronary revascularization, angina pectoris, stroke, intermittent claudication with an ankle-brachial index of 0.9 or less, and peripheral arterial revascularization or amputation due to atherosclerotic disease. In addition, "the member must agree to follow a reduced-calorie diet and increase their physical activity."

Approvals are time-boxed and continuing requires a measured change, with a floor underneath it. "If the clinical criteria for weight management agents are met, initial PA requests may be approved for up to 183 days." For continuation: "Renewal PA requests require the member to have a reduction in BMI compared to their baseline prior to the initiation of the weight management agent. Renewal PA requests for weight management agents may be approved for up to 365 days." The bulletin adds that "All renewal PA requests require the member to be adherent with the prescribed treatment regimen" and that renewals "will not be approved if a member's BMI is below 18.5."

Eight anti-obesity drugs stopped being covered on the same date the new class opened. The bulletin's list is: "Effective July 1, 2026, the following anti-obesity drugs will no longer be covered: Benzphetamine, Diethylpropion, Orlistat, Phendimetrazine, Phentermine, Phentermine/topiramate, Evekeo (for obesity), Saxenda, Xenical." Approvals already granted on the old form are honored "until they expire or until the approved days' supply is used up."

Wegovy is not an adult weight-loss route in Wisconsin, and the bulletin enumerates what it is. "PA requests for Wegovy injection will only be approved for use in the identified clinical conditions: To reduce the risk of major adverse cardiovascular events (MACE) in overweight or obese adults with established cardiovascular disease; To treat metabolic dysfunction-associated steatohepatitis (MASH) in adults; To reduce excess body weight and maintain weight reduction long term in pediatric patients aged 12 years and older with obesity." Wegovy tablets are narrower still: they "will only be approved to reduce the risk of MACE in overweight or obese adults with established cardiovascular disease." The pediatric route requires a BMI "greater than or equal to the 95th percentile standardized by age and gender."

Two scope points the bulletin settles on its own face. It states that "Weight management agents are covered for dual eligibles enrolled in a Medicare Part D Prescription Drug Plan" — the opposite of the carve-out several states apply. And its affected-programs panel names "BadgerCare Plus, Medicaid, SeniorCare" and directs it to a provider list that includes "HMOs and Other Managed Care Programs," so the change is not addressed to fee-for-service alone.

Wisconsin publishes two different hearing windows and which one binds you depends on who issued the notice. The general rule is framed as a bar rather than a deadline: "Review is unavailable if the decision or failure to act arose more than 45 days before submission of the petition for a hearing, except as provided in par. (ag) or (ar)." Where the notice came from a plan, the window is longer: "A person shall request a hearing within 90 days of the date of receipt of a notice from a care management organization or managed care organization upholding its adverse benefit determination." The statute also defers to any different limit set by federal regulation.
