One rule matters more than the rest of this page, so it comes first: never let a date you read here be the only date you act on.
Confirm every deadline against your own notice
Every deadline, dollar figure and coverage rule on this site was read from a named source on a stated date, and it is written down that way so you can check it. It is not a ruling about your plan. Appeal windows and coverage criteria differ by insurer, by plan, by state, by employer, and by the year your plan is written for, and they change during the plan year.
So before you rely on any timing here, do these three things. Read the denial notice you were sent, which is required to state your appeal deadline and where to file. Read your own plan documents, meaning your evidence of coverage, summary plan description, or member handbook. Then call the number on your insurance card and ask the person who answers to confirm the deadline and the address in writing.
If those three disagree with this site, they are right and we are wrong. Act on them, and please tell us so we can fix the page.
By using americanglp.org you accept these terms. If you do not accept them, stop reading and close the page. There is nothing to sign, nothing to cancel, and no account to close, because this site has none of those things.
American GLP is an independent publisher. We read what insurers, state agencies and federal agencies publish about coverage of GLP-1 medication, and we write down what we found, where we found it, and when. Every fact carries its source and its read date so that you can go check it yourself. That is the whole product.
Read this list literally. Each line is a thing readers reasonably assume about a site like this one, and each one is false.
Nobody here is your attorney. Reading this creates no attorney-client relationship and nothing here is privileged. An appeal is a legal process with real consequences, and if yours is complicated, or a deadline is close, get a lawyer or a licensed advocate.
We are not agents, brokers, producers, or advisers of any kind, and we are not licensed to sell or recommend a plan to you. We cannot tell you which plan to buy or whether to switch.

This is a private website. It is not operated by, funded by, endorsed by, or connected to any government body. It is not affiliated with the Centers for Medicare & Medicaid Services, with Medicare, with any state Medicaid agency or managed care plan, or with any state department of insurance. We are not certified Navigators, not a State Health Insurance Assistance Program, and not an ombudsman.
We are not an insurer, a plan administrator, a pharmacy benefit manager, a pharmacy, or a drug manufacturer, and none of them endorses this site. We name them the way any publisher names the subject it writes about. See trademarks.
We cannot file an appeal, request an exception, call your plan, sign anything, or represent you in a review. Those steps are yours to take, and the procedure pages exist to make them easier, not to do them.
You must be 18 or older. The site describes United States coverage rules for readers in the United States. Use it somewhere else if you like, but nothing here is written for another country's system, and no part of it is offered where offering it would be unlawful.
Quote us. Cite us. Send a page to your patient, your client, your caseworker, or your mother. Print a procedure page and take it to an appointment. Read a phone script aloud to your insurer. Use a short excerpt in an article or a brief with attribution to American GLP and a link to the page you took it from. None of that needs our permission and none of it costs anything.
Do not republish pages or substantial parts of them as your own, whether or not you credit us, and whether or not a machine wrote the copy for you. Do not scrape, crawl, or bulk-download the site outside what our robots file permits, and do not hammer it with automated requests. Do not strip a fact from its source line and its read date and present it as current: a dated fact quoted without its date is quoted wrong, and on a deadline that error can cost a reader an appeal. Do not use this site to build a competing coverage database. Do not misrepresent your relationship to us, frame our pages as yours, or remove our attribution.
Every figure on this site is read from a primary document: a payer's published policy, a federal regulation or agency page, a state Medicaid manual, a plan formulary, or a manufacturer's own terms. The page names that source, links to it, and states the month it was read. We do not print a number we could not trace to a document.
That method has limits, and here they are. A source can change the day after we read it. A published policy can differ from how a plan administers it. A state can run rules that its own website describes incompletely. An employer's self-funded plan can override what its insurer publishes. A summary is shorter than the document it summarizes, and the part we left out may be the part that decides your case.
Which is why nothing here is a determination about your coverage, your eligibility, or your appeal rights. Only your plan and the agency reviewing it can make those, and only your own notice states your own deadline. Treat this site as a map of where to look, not as the answer.
Government documents and regulations cited here are public records, and you are free to use them directly. Our summaries, comparisons, tables, phone scripts and procedure pages are not public records. They are our work, and they carry our mistakes as well as our judgment. Do not treat a sentence written here as if a government agency had written it.
We link out constantly, because a fact without its source is not worth much. We do not control any of those sites, we do not vouch for what they say or sell, and their terms and privacy policies apply once you arrive. Some outbound links are partner links that pay us. Those are marked as paid on the page that carries them, and a link is never an endorsement of a company's prices, its clinical practice, or its conduct.

To the fullest extent the law allows, neither American GLP nor anyone who writes for or operates it is liable for any indirect, incidental, special, consequential, exemplary or punitive damages, or for lost savings, lost coverage, denied claims, a missed appeal window, medication costs paid out of pocket, or any other loss arising from your use of this site or your reliance on anything in it. That applies even if we were told such a loss was possible. Where liability cannot be excluded, our total liability for all claims is limited to one hundred United States dollars.
Some states do not allow certain exclusions or limits, so parts of this section may not apply to you. Nothing here limits liability for fraud or for anything else that cannot lawfully be limited.
You agree to indemnify and hold harmless American GLP and the people who operate it from any claim, demand, loss, liability or expense, including reasonable attorneys' fees, arising from your misuse of this site, your breach of these terms, your violation of any law, or your violation of anyone else's rights.
These terms are governed by the laws of the State of Delaware, without regard to its conflict-of-law rules. Any dispute arising from this site or these terms must be brought in the state or federal courts located in Delaware, and you consent to the personal jurisdiction of those courts. If you are a consumer with a mandatory right to sue where you live, this clause does not take that right away.
We update them when the site changes, and the effective date at the top moves when we do. Continuing to use the site after a change means you accept the version posted. We do not apply a change backward to something that already happened.
If any provision is held unenforceable, it is narrowed to the minimum extent needed and the rest stays in force. Our failure to enforce something is not a waiver of it. These terms, together with the privacy page, the funding disclosure, the medical disclaimer and the trademark notice, are the entire agreement between you and us about this site, and they replace anything said before. You may not assign them; we may, if the site changes hands.
