Alabama gives the plainest answer on this site, and it gives it in the one place a reader is most likely to look. Two footnotes on the drug list say that Zepbound is not covered for weight reduction without sleep apnea, and that Wegovy is not covered for weight reduction at all.

Both drugs are nevertheless on the list, which is exactly the trap those footnotes exist to prevent. Seeing a drug on a formulary and concluding you can get it for weight loss is the mistake Alabama has pre-empted in print.

The doors that are open are narrow and each one carries a condition most readers would not predict: for sleep apnea and for liver disease, the criteria require that the patient has no diagnosis of diabetes. These drugs sit in Alabama's antidiabetic class, and the class rules were written around diabetes first.

Alabama also added Wegovy to its list recently rather than removing it, which is worth stating plainly because the direction of travel here is not what a reader following national coverage might assume.

What we could verify for Alabama

The drug list answers the question in two footnotes under Incretin Mimetics: "Zepbound is preferred with clinical criteria for its Obstructive Sleep Apnea (OSA) with obesity indication. Zepbound is non-covered for weight reduction without OSA." And: "Wegovy is available via PA for its cardiovascular and metabolic dysfunction-associated steatohepatitis (MASH) indications. Wegovy is non-covered for weight reduction." The reference tool is footed "Effective 07/01/2026" on every page.

The sleep apnea route has an age floor, two measurements and a diagnosis you must not have: "For OSA indications for incretin mimetics, patient must be ≥18 years of age with moderate to severe obstructive sleep apnea (apnea-hypopnea index [AHI] ≥15) and obesity (BMI ≥30 kg/m2) with no diagnosis of diabetes." The same paragraph requires that "The patient has been counseled to continue a reduced-calorie diet and increased physical activity and is not taking this medication in combination with another GLP-1 agonist", and states that "Stable therapy does not apply."

The liver route repeats the no-diabetes condition and the cardiovascular route asks for weight records at renewal. For MASH, "Requests must include documentation to support the FDA approved indication related to MASH with no diagnosis of diabetes." For cardiovascular renewals, "prescriber must provide documentation of member weight records (dated within the last 90 days), member requires Wegovy® for cardiovascular risk reduction and the benefit of cardiovascular risk reduction outweighs the risk associated with use of GLP-1 agents, and medical records document appropriate diagnosis."

Alabama added Wegovy to the list rather than dropping it, and dated the change. Its quarterly provider ALERT of June 22, 2026 states that "Effective July 1, 2026, the Alabama Medicaid Agency (Medicaid) will" update the list, and records under "PDL Additions" the entry "Wegovy" in "Incretin Mimetics", marked with the code the same document defines: "This agent will be preferred with clinical criteria in place."

These drugs are governed as diabetes drugs, and the class carries a step requirement written for diabetes: "The patient must also have failed 30-day treatment trials with at least two prescribed and preferred antidiabetic agents, either generic, OTC, or brand, within the past 12 months, or have a documented allergy or contraindication to all preferred agents in this class." It appears under the same "Antidiabetic Agents/ Incretin Mimetics" heading as the sleep apnea and liver criteria above.

Alabama publishes one clear filing window and it is measured to arrival, not to postmark: "Unless a different timeframe is prescribed elsewhere by rule, waiver, or statute, a written request for a Fair Hearing or Undue Hardship, if allowed by law, must be received by Medicaid within 60 days from the date the notice of action is mailed." The rule adds that final administrative action follows within 60 days of the request, and "In any event, final administrative action will be taken within ninety (90) days from the date of request for a hearing." The chapter is footed "Revised 7/13/26".
