Answer

Does insurance cover Wegovy for MASH liver disease?

The short answer

Wegovy injection has a MASH indication, but approval does not guarantee payment. Ask for the policy covering noncirrhotic MASH with moderate to advanced fibrosis and submit the required records. An obesity exclusion alone does not settle that coverage question.

MASH is a specific indication, not every fatty-liver finding

FDA approved Wegovy injection for adults with MASH and moderate to advanced liver fibrosis through the accelerated approval pathway. The indication concerns liver disease; it should not be reduced to a general claim that semaglutide is covered whenever a scan shows fat in the liver.

Ask the treating clinician to identify the diagnosis and fibrosis stage in the record. MASH, MASLD, cirrhosis and an isolated abnormal liver test are not interchangeable coverage descriptions. The clinician determines which diagnosis and treatment are appropriate.

Confirm the exact product and policy

Ask the plan for its current Wegovy MASH criteria, effective date and request form. Confirm that the document addresses the injectable formulation and liver indication. Do not substitute Ozempic, oral semaglutide, a compounded product or obesity criteria merely because the names or ingredient overlap.

If the representative points only to a weight-management exclusion, ask whether the plan separately evaluates the approved MASH indication. That question does not establish a right to payment. A plan may still have applicable exclusions, authorization requirements or clinical documentation rules.

VA’s published criteria show why documentation matters

The VA January 2026 MASH criteria call for specialist or locally designated expert involvement and distinguish the evidence required for patients with overweight or obesity from lean patients. The document includes dated fibrosis documentation and exclusion checks; it is not simply a BMI threshold.

Those are VA criteria, not a national rule for commercial insurance or CHAMPVA. Use them as an example of why the exact payer document matters. Ask your own plan which specialist notes, test reports and dates its reviewer needs rather than ordering tests from a general online checklist.

Keep coverage evidence separate from treatment claims

FDA’s accelerated approval is based on biopsy findings from an interim analysis. It requires further evidence to confirm clinically meaningful benefits. Coverage materials should not describe that finding as proof of preventing every liver complication or eliminating future transplant risk.

Your prescriber’s submission should address your actual clinical record and the plan’s criteria. Save the relevant specialist report, the policy version and a copy of what was submitted. Personal concern about liver health cannot replace a documented diagnosis.

If the request is denied, identify the disputed point

Ask whether the decision concerns an excluded benefit, a nonpreferred product, missing evidence, an unmet clinical criterion or the formulation requested. Compare the denial with the correct MASH policy before preparing an exception or appeal.

CHAMPVA’s public guidance lists a Wegovy MASH pathway and requires prior authorization; that does not establish another plan’s coverage. If a request used the wrong indication or form, ask the prescriber and plan how to correct it while preserving any applicable review deadline.
