Answer

What do insurers require for Wegovy prior authorization?

14 documents cited · Read September 2026

The short answer

Most criteria want a BMI of 30, or 27 with a weight-related condition. You'll also need a diet and exercise record. Renewal usually needs 5% weight loss. Heart-risk use asks for a BMI of 27 and past heart disease.

What does a Wegovy prior authorization ask for?

Expect three things: a starting BMI, a record of diet and exercise, and your prescriber's request.

Most set the BMI floor at 30, or 27 with a condition like high blood pressure. Aetna's BMI 35 policy and Cigna's IP0739 raise the main floor to 35.

The pill and the pen can differ. Caremark approves the tablet for 6 months first, and the adult injection for 8 months.

Cigna moved Wegovy into its own policy, IP0814, effective September 1, 2026. UnitedHealthcare's program only applies where your employer chose to cover weight-loss drugs.

How do the big insurers and PBMs compare?

Here's what each published document asks for when Wegovy is prescribed for weight management.

What if Wegovy is prescribed to lower your heart risk?

That's a separate FDA-approved use with separate criteria. A plan that excludes weight-loss drugs may still cover it.

UnitedHealthcare runs a Wegovy program for exactly those plans. It requires age 45 or older and medical records showing past heart disease.

On Medicare, Novo Nordisk says this use is covered through standard Medicare Part D benefits.

Wegovy to reduce heart-attack and stroke risk: published criteria
Insurer (document)Heart history requiredStarting BMIOther conditionsApproval
CVS Caremark (4774-C)Prior heart attack, stroke, artery disease or revascularization27No type 2 diabetes; on standard heart treatment or a reason not to be12 months

Kaiser Permanente NorthwestHistory of STEMI or Type 1 NSTEMI27Age 55 to 74; no diabetesNot stated

These describe what each document requires. Only your prescriber can say whether your history fits them.

What records should your prescriber send?

Send proof of each test the criteria name. Caremark and Aetna tag every BMI line as needing documentation.

Novo Nordisk's own insurance guide suggests you ask the plan which documents it needs. It gives weight, BMI, past treatments and lab results as examples.

  • Your starting BMI, measured before any GLP-1 drug.
  • Any weight-related condition the criteria list, such as high blood pressure.
  • Notes from your diet, exercise or weight management program.
  • For heart-risk use, records of the past heart event the criteria name.
  • For a renewal, your weight loss since baseline.

What do Express Scripts and Optum Rx publish?

Express Scripts doesn't post its criteria on its prior authorization page. It says they're available upon request at 800.753.2851.

Its 2026 National Preferred Formulary lists the Wegovy pens, Wegovy HD and Wegovy tablets.

Optum Rx lists Wegovy as needing prior authorization, limited to 4 syringes per 28 days. If you can't wait, Optum says to ask for a supply of 5 days or less. You'll pay the full cost for it.

What does Novo Nordisk offer for the paperwork?

Its insurance guide gives you questions to ask your plan, starting with a request for the written criteria.

For prescribers, NovoCare links to CoverMyMeds, a no-cost electronic prior authorization tool. Its coverage check can pre-fill that form.

What should you do if Wegovy is denied?

Find out why first. A missing record can be sent again, but a plan exclusion isn't a paperwork problem.

  1. Read the denial letter and note the exact criterion it says you missed.

  2. Ask the plan for the written criteria it used for your request.

  3. Have your prescriber resend the missing record with a new request.

  4. If it's still denied, file an internal appeal before your plan's deadline.

  5. If your employer excludes Wegovy, ask your benefits office to add coverage.

What to say to your plan or pharmacy benefit manager

I'm calling about prior authorization for Wegovy. Can you send me the written criteria my plan uses, for weight management and for heart-risk reduction? Which records does my prescriber need to send? How long does a first approval last, and what's needed for renewal?

Write down the date, the representative's name and any reference number. Ask whether the pill and the pen have different criteria.
