Answer

Zepbound prior authorization: what insurers require

11 documents cited · Read September 2026

The short answer

Most criteria want a BMI of 30, or 27 with a weight-related condition. You'll also need a diet and exercise record. Aetna's BMI 35 policy sets a higher bar. Renewal usually needs 5% weight loss.

What does every Zepbound prior authorization ask for?

The criteria differ in detail, but they share a spine. You'll need a starting BMI, a diet and exercise record, and your prescriber's request.

Most set the BMI floor at 30, or 27 with a condition like high blood pressure. Aetna's BMI 35 policy sets its floor at 35 instead.

Starting BMI means before any GLP-1 drug. Cigna says so outright, so switching from Wegovy doesn't reset your baseline.

None of this matters if your plan excludes weight-loss drugs. UnitedHealthcare calls its program optional, for employers that choose to cover them.

How do the big insurers and PBMs compare?

Here's what each published document asks for when Zepbound is prescribed for weight management.

Zepbound for weight management: published prior authorization criteria
Insurer or PBM (document)Starting BMILifestyle requirementFirst approvalRenewal needs
CVS Caremark (6192-C)30, or 27 with a weight-related conditionAt least 6 months in a weight management program first8 months5 percent weight loss, or keeping it off

Aetna (6450-C, BMI 35 policy)35At least 6 months in a weight management program first8 months5 percent weight loss, or keeping it off
Kaiser Permanente Northwest30, or 27 with a listed conditionActive diet or exercise changes, or a referral12 months, after a 6-month semaglutide trial5% weight loss, checked every 12 months

Read at each publisher's own document. Kaiser's criteria carry an effective date of November 5, 2026. Your plan's own documents can differ from these templates.

What records should your prescriber send?

Send proof of each test the criteria name. Caremark and Aetna tag every BMI line as needing documentation.

Kaiser wants your weight and BMI recorded within about the last 30 days. Lilly's own prior authorization guide lists the same core records.

  • Your starting BMI, measured before any GLP-1 drug.
  • Any weight-related condition the criteria list, such as high blood pressure.
  • Notes from your diet, exercise or weight management program.
  • For a renewal, your weight loss since baseline and time on a stable dose.

What about Zepbound for sleep apnea?

Sleep apnea is a separate use with its own criteria. Caremark approves it for 6 months first, then 12 months at a time.

Cigna approves it for 1 year and wants 10% weight loss at renewal. The sleep apnea page compares each insurer's rules side by side.

What do Express Scripts and Optum Rx publish?

Express Scripts doesn't post its criteria on its prior authorization page. It says they're available upon request at 800.753.2851.

Its 2026 exclusions list names the Zepbound KwikPens and vials, with Zepbound pens as a preferred alternative.

Optum Rx lists Zepbound as needing prior authorization, limited to 4 syringes per 28 days. If you can't wait, Optum says to ask for a supply of 5 days or less. You'll pay the full cost for it.

Do you always need prior authorization?

No. Lilly says no prior authorization is needed for the vial or KwikPen through LillyDirect. You must certify it was prescribed for an approved use.

Caremark says it will add Zepbound back to its commercial formularies October 1, 2026. Your employer can still customize what its plan covers.

What should you do if Zepbound is denied?

Find out why first. A missing record can be sent again, but a plan exclusion isn't a paperwork problem.

  1. Read the denial letter and note the exact criterion it says you missed.

  2. Ask the plan for the written criteria it used for your request.

  3. Have your prescriber resend the missing record with a new request.

  4. If it's still denied, file an internal appeal before your plan's deadline.

  5. Your prescriber's office can call 1-800-LillyRx for help with prior authorization paperwork.

What to say to your plan or pharmacy benefit manager

I'm calling about prior authorization for Zepbound for weight management. Can you send me the written criteria my plan uses? Which records does my prescriber need to send, and how long does a first approval last? What weight loss do you need to see before renewal?

Write down the date, the representative's name and any reference number. If it's denied, ask which criterion wasn't met.
