Answer

Does your PBM cover Zepbound?

The short answer

Maybe. Starting October 1, 2026, CVS Caremark adds Zepbound back as a preferred drug on its template formularies, but your employer can still customize coverage. Express Scripts and Optum Rx already list the Zepbound pen.

What did CVS Caremark change on October 1, 2026?

It put Zepbound back. On July 1, 2025, Caremark preferred Wegovy on its commercial template formularies and removed Zepbound. If your prescription got switched that summer, that's why.

On May 28, 2026, CVS Health announced Zepbound's return "as an additional preferred option October 1, 2026." Caremark's October 2026 Standard Control list shows it: Zepbound sits in the antiobesity class beside Wegovy, Wegovy HD, Foundayo and Qsymia. The July 2026 list still excluded it.

Lilly announced the same day that current Zepbound patients keep their coverage, and that access would broaden across template plans by October 1.

Does the change apply to your plan?

Not automatically. A PBM's formulary is a template, and your employer or health plan decides how much of it to adopt. CVS's own announcement says so: plan sponsors that use Caremark's template formularies "retain discretion to customize coverage for their members."

Two separate questions decide your answer. First, does your plan cover weight-loss drugs at all? That's a benefit design choice, and many employer plans exclude the whole category. Second, if it does, which Caremark list does it follow?

Caremark’s October 2026 Advanced Control and Value editions now also list Zepbound and Foundayo. We checked the newly published documents in October. Your plan must still adopt the relevant template and cover the weight-loss benefit; the public list alone cannot confirm either.

If your employer pays claims itself, it's a self-funded plan, and it can shape its formulary more freely than an insured plan. Ask HR which kind you have.

Were you switched to Wegovy in 2025? Can you switch back?

You may be able to, if your plan adopts the change. In 2025, Caremark told members that an active Zepbound prior authorization carried over when they moved to Wegovy, so nobody had to start the approval again.

The 2026 announcement doesn't say whether that works in reverse. Don't assume your old Wegovy approval covers Zepbound. Ask Caremark, or your prescriber's office, whether a new prior authorization is needed before your next fill.

If Zepbound still isn't on your plan's list after October 1, you can still ask for it. Caremark's 2025 guidance said a member who had severe side effects on Wegovy, or didn't lose enough weight, could request a formulary exception for a different weight-loss drug.

What do Express Scripts and Optum Rx list?

Both already list a Zepbound product for 2026, but only one form of it. Each also says your own plan decides whether weight-loss drugs are covered at all.

  • Express Scripts National Preferred Formulary (exclusions list dated July 1, 2026): Zepbound KwikPens and Zepbound vials are excluded. The preferred alternatives are liraglutide, Foundayo, Wegovy HD, Wegovy pens, Wegovy tablets and Zepbound pens.
  • Optum Rx Premium Standard formulary (effective July 1, 2026): the Zepbound auto-injector and Wegovy are tier 2 with prior authorization. The Zepbound KwikPen and the Zepbound vial are marked excluded.
  • Optum Rx Select Standard formulary (effective July 1, 2026): the Zepbound auto-injector and Wegovy are tier 2 with prior authorization.
  • On both Optum lists, Wegovy and the Zepbound auto-injector carry a benefit-design flag, which the legend explains as coverage "determined by your prescription medication benefit plan."

How do you find out which list you're on?

It takes about five minutes, and it beats guessing from news reports. Work through these in order.

  1. Find your PBM. It's printed on your insurance card, usually beside "Rx" or "pharmacy." Your health insurer and your PBM are often different companies.

  2. Log in to the PBM's member site or app and search for Zepbound. Look at the result for your plan, not the public list.

  3. Check the exact form. Express Scripts and Optum Rx treat the Zepbound pen differently from the KwikPen and the vial.

  4. Note whether it says prior authorization, step therapy or a quantity limit. Each one is a separate hurdle.

  5. If Zepbound shows as not covered, ask HR whether your plan excludes weight-loss drugs or just follows a list that excludes Zepbound. The fix is different for each.

What if you're still denied after October 1?

Read the denial letter for the reason. A plan exclusion and a formulary decision are different, and they're appealed differently.

If your plan covers weight-loss drugs but not Zepbound, a formulary exception is the usual route. If your plan excludes the category, ask HR whether an exception or a plan change is possible at open enrollment. If you're denied on medical necessity, you can appeal, and the steps for a commercial or employer plan are on the procedures page.

What to say to HR or your PBM

My plan's pharmacy benefit is run by [PBM name]. Starting October 1, 2026, CVS Caremark added Zepbound back to its template formularies. Does my plan cover weight-loss medications, and which formulary does it follow? Is Zepbound covered on my plan, and in which form? Do I need a new prior authorization, or does my current one carry over?

Ask for the answer in writing, and write down the date and the name of the person you spoke with. Open enrollment is the time to raise a plan-level exclusion.
