Answer
Where is brand Zepbound cheapest without insurance?
The short answer
LillyDirect, TrumpRx and a retail pharmacy using Lilly's self-pay card all charge $299 to $449 a month, by dose. Costco's own member price is higher. Refill within 45 days, or the top doses cost $699.
What does Zepbound cost at each channel this month?
The cheapest legal price for brand Zepbound is Lilly's own self-pay price, and three of the channels below are just different ways to reach it. The price depends on your dose, not on where you click Buy.
| Channel and form | 2.5 mg | 5 mg | 7.5 mg | 10 to 15 mg |
|---|---|---|---|---|
| LillyDirect: KwikPen or vial | $299 | $399 | $449 | $449 |
| TrumpRx: vial, filled by LillyDirect | $299 | $399 | $449 | $449 |
| Retail pharmacy with Lilly's self-pay card: KwikPen | $299 | $399 | $449 | $449 |
| Costco member price: KwikPen, no Lilly card | $499.22 | $499.22 | $499.22 | $699.21 |
Read September 2026. The $449 price at 7.5 mg and up holds only if you refill within 45 days; otherwise it's $499 at 7.5 mg and $699 at 10, 12.5 and 15 mg. A month is 28 days.
What's the catch in the $449 price?
Timing. From 7.5 mg up, you get $449 on your first purchase and on every refill made within 45 days. Let a refill slip past 45 days and you pay the regular price: $499 at 7.5 mg and $699 at 10, 12.5 and 15 mg.
At the 2.5 mg and 5 mg doses, the price is the same either way: $299 and $399.
Is TrumpRx cheaper, or the same price through a different door?
It's the same price through a different door. TrumpRx lists the Zepbound vial at $299, $399 and $449 by dose, and says Lilly handles every vial order through LillyDirect. TrumpRx itself doesn't sell or dispense anything.
The "original price" of $1,087.00 printed beside each strength isn't a price you'd pay anywhere on this list. TrumpRx pricing is for cash-paying patients only.
Do GoodRx or Costco beat Lilly's price?
GoodRx doesn't beat it; it shows it. For the 2.5 mg KwikPen, GoodRx lists $299.00 at CVS, Walgreens, Walmart, Kroger, Costco and others, and labels it a manufacturer offer. That's Lilly's self-pay price, not a separate GoodRx discount.
Costco's own member price is higher: $499.22 for a KwikPen at 2.5, 5 or 7.5 mg, and $699.21 at the top three doses. If you fill at Costco, bring Lilly's self-pay KwikPen card rather than relying on the member price.
Vial or KwikPen: does the form change the price?
Not at Lilly's price. The vial and the KwikPen cost the same at every dose. What changes is where you can buy them. The vial is sold only through LillyDirect. The KwikPen is sold through LillyDirect with home delivery or pickup at Walmart Pharmacy, and at other pharmacies with Lilly's self-pay card.
The single-dose pen is the expensive form if you pay cash. LillyDirect shows no self-pay price for it, Lilly's self-pay card doesn't apply to it, and Costco lists four pens at $1,086.56.
Does paying cash count toward your deductible?
Not under Lilly's self-pay card. Its terms have you agree not to seek reimbursement from any insurer or government program and not to apply the cost toward a deductible or out-of-pocket limit. If that matters to you, ask your plan how it treats a cash purchase before you buy.
Have insurance that excludes Zepbound?
Then compare two things before you pay: Lilly's savings card for commercially insured patients, and a formulary exception from your plan. The card's price without coverage matches the self-pay prices above for the KwikPen. A formulary exception, if your plan grants one, usually beats both.
What to ask the pharmacy
I'm paying cash for the Zepbound KwikPen with Lilly's self-pay savings card. What's my price at this dose after the card? Is this my first fill or a refill within 45 days of the last one?
If the pharmacy quotes you its own cash price, ask it to run the Lilly card. Write down the price you're quoted and the date.
