Answer
GLP-1 authorization renewal and maintenance coverage
The short answer
Renewal can require proof of response and continued treatment, even after an initial approval. Keep the original baseline, current clinical record and approval end date. Use the continuation criteria for your exact plan and indication.
Read the continuation section, not only the entry criteria
Initial approval and renewal are separate parts of a policy. A plan may ask whether you met an initial eligibility threshold before treatment, then require evidence that treatment is helping and is being continued appropriately.
Keep the authorization number, effective dates and the policy version used for approval. Ask whether your prescriber should file a continuation request or a new initial request after a plan change or treatment interruption. There is no universal GLP-1 renewal checklist.
A lower current BMI is not the same as a missing baseline
Weight-management records should distinguish the measurement before treatment from the current measurement. The point is to show what happened over time rather than replacing the original starting point with whichever number is easiest to find.
In the Caremark Zepbound policy read for this article, weight-management continuation allows maintained initial weight loss and requires a period at a stable maintenance dose. That is a specific policy example, not a promise for every Caremark-administered employer benefit.
Diagnosis changes the evidence the plan asks for
Cigna’s September 2026 Zepbound policy distinguishes weight-management continuation from obstructive sleep apnea. The evidence sought for one pathway cannot simply be borrowed for another. Ask which indication the existing approval actually covers.
The prescriber should explain the clinical response using truthful records. Do not manufacture a diagnosis or describe an adverse event as a contraindication without clinical support. A coverage document is not a substitute for an individual medical assessment.
The renewal may also change who can submit it
UnitedHealthcare’s February 2026 notice for certain Georgia State Health Benefit Plan HMO and HDHP members says a designated program becomes the prescribing route when their existing weight-loss authorization expires. It excludes diabetes GLP-1s from that specific change.
This illustrates why an old authorization is not the whole renewal process. It is not a rule for every Georgia resident or UnitedHealthcare member. Check your own notice for program, provider or submission changes before the clinic sends a request through the former route.
Prepare the record before the approval ends
Ask the clinic to locate baseline measures, current response, treatment dates, relevant follow-up and the current policy. Record when the request was submitted and whether the plan has everything needed. If it is refused, obtain the exact unmet criterion and appeal instructions.
A maintained response and a completed authorization request are different facts. If the benefit has changed or the medication is excluded, address that separately. Ask the prescriber about continuity of treatment while coverage is being resolved.
Every claim above, with the document behind it
If something here decides anything for you, read the source rather than our summary of it. These are the documents we opened, and the month we opened them.
Caremark Zepbound weight-management continuation requires at least three months at a stable maintenance dose and a loss or maintained loss of at least 5% of baseline weight.

American GLP is not a law firm, an insurer, a government office or a clinician, and nothing here is legal or medical advice. This page describes what published rules say. Your own plan documents and your own notice govern your case, and rules change — check the sources before you act.
