Blue Cross and Blue Shield of North Carolina

Blue Cross and Blue Shield of North Carolina · Drug benefit: Not named in the documents read · 2 documents cited · Read September 2026

Requires anti-obesity drug coverage on your plan, 6 months of lifestyle change first, and 5% weight loss to renew Wegovy, Zepbound or Foundayo.

Blue Cross NC's anti-obesity policy makes the benefit a criterion: to get Wegovy, Zepbound, Foundayo or Saxenda approved, you must have anti-obesity medication coverage. If your formulary excludes the drug, a non-formulary exception is the route.

Then the clinical bar. A BMI of 30, or 27 with a weight-related condition; a BMI of 25 counts for people of South Asian, Southeast Asian or East Asian descent. Your doctor also has to show you couldn't lose about a pound a week over 6 months with diet and activity, unless a weight-related condition needs treatment now.

The first approval lasts 180 days. To renew for a year, you need to have lost and kept off at least 5% of your starting weight.

What changed in 2026?

The policy dated July 2026 removed the step requirement for Foundayo. Earlier in 2026 Blue Cross NC added Foundayo, Wegovy HD and the Wegovy tablet to the policy.

Does it cover Zepbound for sleep apnea?

Yes, as a separate path: moderate to severe sleep apnea, a BMI of 30 or more, a sleep test, and an apnea-hypopnea index of 15 or more at baseline.

What these documents cannot tell you

Everything above is what Blue Cross and Blue Shield of North Carolina publishes. None of it establishes what your own plan does, and the gap between those two things is wider than a carrier policy makes it look.

  • Whether your plan includes anti-obesity medication coverage isn't in the policy. Your benefit booklet or HR can tell you.
  • A self-funded employer that carves its pharmacy benefit out to another PBM isn't bound by this policy.
  • The policy doesn't say which Blue Cross NC formularies list each drug.
  • None of this is legal or medical advice. Your plan documents and your notice govern your case.
