Premera Blue Cross
Covers weight-loss GLP-1s only when the employer elects the benefit, then approves Wegovy for 7 months and renews at 5% weight loss.
Premera, the Blue plan for Washington and Alaska, puts the benefit question first in its own words: weight-loss GLP-1s are covered only when an employer group elects the benefit. If your group didn't, they aren't covered for obesity.
If it did, Premera's policy 5.01.621 sets the rules: three months of diet and behavior change, a BMI of 30 or 27 with a weight-related condition, a 7-month first approval for Wegovy or Foundayo, and a 5% weight-loss test to renew.
Does your plan's BMI rule match the policy?
Not always. Premera says some plans set their own BMI threshold for these drugs, and it can differ from the policy's. So the number in the policy is a default, not a promise.
What the drug list shows
Premera's 2026 drug list carries Foundayo, Saxenda, Wegovy, Wegovy HD, the Wegovy tablet and Zepbound on tier 3 marked PA and OPT, which the list defines as an optional benefit. The same list says many plans exclude weight-management drugs.
Every claim above, with the document behind it
These are the policies we opened and the month we opened them. Each one carries its own policy number and effective date on its face, and those are printed here as the document shows them.
"Weight management GLP-1s are reviewed under the weight-loss drug policy and are only covered when an employer group elects this benefit." And: "If a group doesn't elect weight-loss drug coverage, GLP-1s for obesity are not covered."

What these documents cannot tell you
Everything above is what Premera Blue Cross publishes. None of it establishes what your own plan does, and the gap between those two things is wider than a carrier policy makes it look.
- Whether your employer elected the weight-loss benefit isn't public. Your benefit booklet or HR can tell you.
- Your plan's own BMI threshold may differ from the policy's, and Premera says so.
- A self-funded employer plan administered by Premera can set its own rules.
- None of this is legal or medical advice. Your plan documents and your notice govern your case.
