Covers weight-loss drugs only where the benefit doesn't exclude them, then requires 5% weight loss to renew Wegovy, Zepbound and Foundayo.

Independence Blue Cross puts the benefit question in its policy's first line: weight-loss drugs aren't covered under the pharmacy benefit unless your benefit doesn't exclude them. The criteria only apply to groups that kept the coverage.

For those groups, its weight-loss drug policy asks for a BMI of 30, or 27 with a weight-related condition, and a first approval of 6 months. To stay on Zepbound or Foundayo after 6 months, you need 5% weight loss; for Wegovy the test comes at 7 months.

The policy also covers Wegovy for heart risk and MASH, and Zepbound for sleep apnea, under separate criteria.

Which formulary do these rules cover?

Independence says its posted pharmacy policies apply to the Select and Value formularies. Its Premium formulary is a separate list: it carries Wegovy and the Zepbound auto-injector on tier 2 with prior authorization, marks them as a benefit design option, and excludes the Zepbound KwikPen.

What does the sleep apnea rule require?

For Zepbound and sleep apnea, records must show a BMI of 30 or more and a sleep study with 15 or more breathing events an hour. To renew, you need 5% weight loss in the first 6 months.

Every claim above, with the document behind it

These are the policies we opened and the month we opened them. Each one carries its own policy number and effective date on its face, and those are printed here as the document shows them.

"Weight loss medications are not covered under the pharmacy benefit except for members whose benefit does not exclude weight loss drugs." And: "This Claim Payment Policy is available to those groups that do not exclude weight loss drugs."

What these documents cannot tell you

Everything above is what Independence Blue Cross publishes. None of it establishes what your own plan does, and the gap between those two things is wider than a carrier policy makes it look.

  • Whether your group excludes weight-loss drugs isn't public. Your benefit booklet or HR can tell you.
  • Independence says the posted policies apply to its Select and Value formularies. If you're on the Premium formulary, other criteria may apply.
  • A self-funded employer plan administered by Independence can set its own rules.
  • None of this is legal or medical advice. Your plan documents and your notice govern your case.
