Carrier
Florida Blue
Excludes FDA-approved weight-loss drugs unless a plan buys the coverage separately; its new Wegovy HD, KwikPen and Foundayo rules apply only to those groups.
Florida Blue starts from no. Its list of drugs it doesn't cover says drugs FDA-approved for weight loss aren't covered on your policy unless the coverage was purchased separately.
Its medication guide says the same thing from the other side: a drug isn't covered for a weight-loss use, and your Schedule of Benefits has the details. So the first question is whether your employer bought weight-loss coverage.
If it did, the newest rules are in Florida Blue's pharmacy bulletin. From October 1, 2026, Foundayo, Wegovy HD and the Zepbound KwikPen need prior authorization and have quantity limits, but only for self-insured groups that purchased that coverage.
What changes on October 1, 2026?
Foundayo, Wegovy HD and the Zepbound KwikPen join the prior authorization program and the quantity limit program. Wegovy HD is capped at 4 pens every 28 days and the KwikPen at 1 pen every 28 days.
Every one of those lines carries the same footnote: the rule applies only to self-insured groups that purchased weight-loss coverage.
Is Wegovy on the medication guide?
No. Wegovy, Zepbound and Saxenda don't appear on the Open Medication Guide dated July 2026, while Ozempic does. That matches the exclusion: a drug your policy doesn't cover for weight loss isn't on the list of covered drugs.
Every claim above, with the document behind it
These are the policies we opened and the month we opened them. Each one carries its own policy number and effective date on its face, and those are printed here as the document shows them.
"Drugs FDA approved for weight loss, cosmetic use or lifestyle modification are not included in this list. They are not covered on your policy unless specifically purchased separately."

What these documents cannot tell you
Everything above is what Florida Blue publishes. None of it establishes what your own plan does, and the gap between those two things is wider than a carrier policy makes it look.
- Whether your employer purchased weight-loss coverage isn't in these documents. Your Schedule of Benefits or HR can tell you.
- The October 1, 2026 rules are written for self-insured groups that bought the coverage. They don't describe a fully insured plan.
- The clinical criteria Florida Blue applies aren't published in these documents. It says a summary comes with its authorization forms.
- None of this is legal or medical advice. Your plan documents and your notice govern your case.
