Covers Wegovy and the Zepbound auto-injector only for groups that elect GLP-1 coverage, excludes the Zepbound KwikPen, and was still reviewing Foundayo.
Blue Cross and Blue Shield of Illinois marks Wegovy and Zepbound on its Performance Drug List as group-specific: whether you're covered depends on your employer's plan. Its own list also notes that some plans exclude weight-loss drugs entirely.
Its May 2026 update for self-funded groups calls GLP-1 coverage for weight management optional. For groups that elect it, the Zepbound auto-injector stays covered and the Zepbound KwikPen, sold only direct to consumers, is excluded. Foundayo was under review for a spot on the list, with coverage exception requests open.
What the Performance Drug List shows
The July 2026 list shows Wegovy injections, Wegovy HD and the Zepbound auto-injector as preferred brands marked group-specific, each with prior authorization and a limit of 4 pens every 28 days. Foundayo isn't on it.

What these documents cannot tell you
Everything above is what Blue Cross and Blue Shield of Illinois publishes. None of it establishes what your own plan does, and the gap between those two things is wider than a carrier policy makes it look.
- Whether your group elected GLP-1 coverage isn't public. Your benefit booklet or HR can tell you.
- The spring 2026 update is written for self-funded groups; a fully insured plan can differ.
- The clinical criteria Prime applies weren't published where they could be read.
- None of this is legal or medical advice. Your plan documents and your notice govern your case.
